International students requesting the Form I-20 from UCLA must complete the I-20 request by 5PM, May 3rd.

The I-20 Request required documents are the following:

• Scanned copy of the biographical page in your passport that shows your full name, passport number, birth date, expiration date and photograph
• Confidential Financial Statement (CFS) and Proof of Sufficient Funds
• Proof of English Proficiency
